Why specialised rehabilitation remains essential in modern haemophilia care
Despite major advances in haemostatic treatment and widespread use of primary prophylaxis, musculoskeletal complications remain a significant clinical burden in people with haemophilia (PWH). Joint deterioration, chronic pain and functional limitations continue to affect a substantial proportion of adult patients.
Long-term cohort studies show that joint health may deteriorate over time despite low bleeding rates, particularly in ankle and elbow joints. Approximately 37% of adults demonstrate clinically relevant joint deterioration over a 5–10 year period, with disease severity, joint bleeds and synovitis as key predictors.
Modern haemophilia management requires not only optimal haematological treatment, but also structured, specialised musculoskeletal rehabilitation to preserve function and prevent further disability.
The programme is coordinated by a rehabilitation physician and delivered by a multidisciplinary team with extensive experience in haemophilia-related musculoskeletal disorders. All interventions are performed in close coordination with the patient’s haematology team, ensuring clinical safety and continuity of care.

How the rehabilitation programme works
Each patient undergoes a comprehensive medical and functional assessment, followed by a personalised rehabilitation plan that may include:
Physical rehabilitation
- Joint-specific physiotherapy, progressive mobilisation and strengthening, neuromuscular and balance training, functional re-education, hydrotherapy and individualised exercise programmes.
Pain and musculoskeletal management
- Chronic pain management strategies, myofascial techniques, management of stiffness and contractures, and ultrasound-guided interventions when indicated.
Education and prevention
- Patient and family education, safe physical activity guidance, bleeding risk awareness during exercise and long-term joint protection strategies.
All interventions are coordinated with the patient’s haemostatic treatment schedule andperformed following administration of clotting factor or non-replacement therapy, ensuring maximum clinical safety.

Stay and treatment at Medifit Syncrosfera, Dénia
During the rehabilitation programme, patients stay on a full-board basis at the Medifit La Sella Center in Dénia, where treatment and accommodation are fully integrated in a structured clinical setting.
Rehabilitation takes place in modern treatment rooms, exercise areas and an indoor therapy pool, allowing for a consistent daily routine under continuous medical and physiotherapy supervision. The calm natural surroundings support focus, recovery and adherence to the programme.
The programme is designed to address rehabilitation in a comprehensive way, supporting both physical recovery and overall balance. Patients are guided in structuring their activities, managing physical load and recovery, and regaining confidence in daily life, always adapted to their individual condition and capabilities.
Family accompaniment is possible, providing additional emotional support during the rehabilitation stay.
